The epistemic basic structure of a society consists of those institutions that have the greatest impact on individuals’ opportunity to obtain knowledge on questions they have an interest in as citizens, individuals, and public officials. It plays a central role in the production and dissemination of knowledge and in ensuring that people have the capability to assimilate this knowledge. It includes institutions of science and education, the media, search engines, libraries, museums, think tanks, and various government agencies. This article identifies (...) two demands of justice that apply specifically to the institutions that belong to it. First, the epistemic basic structure should serve all citizens fairly and reliably. It should provide them with the opportunity to acquire knowledge they need for their deliberations about the common good, their individual good, and how to pursue them. Second, the epistemic basic structure should produce and disseminate the knowledge that various experts and public officials need to successfully pursue justice and citizens need to effectively exercise their rights. After arguing for these duties, I discuss what policies follow from them and respond to the worry that these duties have illiberal implications. (shrink)

In this article we develop an account of justice in the distribution of knowledge. We first argue that knowledge is a fundamental interest that grounds claims of justice due to its role in individuals’ deliberations about the common good, their personal good and the pursuit thereof. Second, we identify the epistemic basic structure of a society, namely, the institutions that determine individuals’ opportunities for acquiring knowledge and discuss what justice requires of them. Our main contention is that a systematic lack (...) of opportunity to acquire knowledge one needs as an individual and a citizen because of the way the epistemic basic structure of a society is organized is an injustice. The news media is a critical source of information for the public. However, it neglects the interests of the poor. In this paper, we explore why this happens, why it matters, and what might be done about it. As to why this happens, we identify two main reasons: because of the way that media is funded and because of the composition of its journalists and its sources. As to why this matters, we argue that this neglect is problematic for three (...) main reasons: it deprives the poor of vital information; it contributes to public support for or acceptance of policies that harm the interests of the poor; and it undermines political equality. And as to what might be done about it, we suggest two proposals: proposals that change the composition of who makes the news and proposals that change how the media is funded. (shrink)

The democratization of science entails the public having greater influence over science and that influence being shared more equally among members of the public. This chapter will present a thumbnail sketch of the arguments for the democratization of science based on the importance of collectively shaping science’s impact on society, the instrumental benefits of public participation in science, and the need to ensure that the use of science in politics does not undermine collective self-government. We provide an analysis of the public's having warranted epistemic trust in science, that is, the conditions under which the public may be said to have well-placed trust in the scientists as providers of information. We distinguish between basic and enhanced epistemic trust in science and provide necessary conditions for both. We then present the controversy regarding the connection between autism and measles–mumps–rubella vaccination as a case study to illustrate our analysis. This article develops an account of distributive epistemic justice in the production of scientific knowledge. We identify four requirements: (a) science should produce the knowledge citizens need in order to reason about the common good, their individual good and pursuit thereof; (b) science should produce the knowledge those serving the public need to pursue justice effectively; (c) science should be organized in such a way that it does not aid the wilful manufacturing of ignorance; and (d) when making decisions about (...) epistemic risks, scientists should make sure that there aren’t social groups or weighty interests that are neglected. After discussing these requirements, we examine the relationship between discriminatory and distributive epistemic injustice in science and argue that they often compound each other. (shrink)

Building, restoring and maintaining well-placed trust between scientists and the public is a difficult yet crucial social task requiring the successful cooperation of various social actors and institutions. Kitcher’s takes up this challenge in the context of liberal democratic societies by extending his ideal model of “well-ordered science” that he had originally formulated in his. However, Kitcher nowhere offers an explicit account of what it means for the public to invest epistemic trust in science. Yet in order to understand how (...) his extended model and its implementation in the actual world address the problem of trust as well as to evaluate it critically, an explicit account of epistemic public trust in science needs to be given first. In this article we first present such an account and then scrutinize his project of building public trust in science in light of it. We argue that even though Kitcher’s ideal model and his proposals for its implementation in the real world face a number of problems, they can be addressed with the resources of our account. There are limits on the duty to tell the truth. Sometimes, because of the undesirable consequences of honesty, we are morally required not to reveal certain truths and can even be required to lie. In this article, we explore the implications of this uncontroversial claim for the practice of political philosophers. We argue that, given the consequences of misunderstandings and misrepresentations that might occur, political philosophers will sometimes be under a moral duty not to disseminate their research and, in highly (...) exceptional cases, have a moral duty to lie outright. Faik Kurtulmuş’s exploration of the epistemic basic structure (EBS) invites us to think about the generation, dissemination, and absorption of knowledge in a society, emphasizing the role of institutions in determining epistemic outcomes. Moreover, Kurtulmuş—in joint work with Gürol Irzık—offers a normative take on the EBS from the viewpoint of the theory of justice and does not shy away from drawing specific policy recommendations. Thus, a powerful, innovative concept is used to extend an influential theory and draw (...) out its practical implications. What more is there to wish for? A project of such ambition inevitably faces serious challenges. I address the issue of the alignment between the ideal and non-ideal elements of Kurtulmuş’s theory with an eye toward the practicalities of social reform. My central claim is that the philosophers striving to promote distributive epistemic justice need to think carefully about whom they intend to address with their proposals and how to persuade such addressees. I also maintain that emphasis on a holistic approach to social reform represents a major hindrance to its pragmatic success. (shrink)

Objective: Kindling is an animal model of epilepsy induced by electrical stimulation of the brain. The present study aimed to present a different perspective with a bibliometric approach by using the literature data on the “Kindling model” related keywords in the Web of Science (WoS) online database between 1980 and 2023. -/- Methods: The bibliometric data were obtained from the online database WoS and analyzed and visualized with the VoS Viewer Program. The bibliometric datasets were analyzed and visualized regarding article (...) productivity numbers according to years, article productivity numbers according to countries, the most used keywords according to authors, and cross-country cooperation. -/- Result: Considering the results of the analysis of the published datasets, 2022 was determined as the year with the highest article productivity, and an acceleration was observed in the publication increase rate on the subject in general. When the order of the countries in the top three in the number of article productivity was examined, the USA, Germany, and Japan are the main countries, respectively. The most used keywords by the authors were determined as “Epilepsy”, “Kindling”, and “Hippocampus”. In the cooperation among countries, it was found that the USA, Germany, and Japan had more cooperation with other countries, respectively. -/- Conclusıon: This study will contribute to the literature by providing a detailed understanding of the research basis, relevant research results, current research boundaries and main research focus in the Kindling Model. How can we understand the phenomena of loss and mourning in the Heideggerian framework? There is no established interpretation of Heidegger that gives an elaborate account of the phenomena of loss and mourning, let alone gauges its importance for our understanding and assessment of authentic existence in Heidegger. This paper attempts to do both. First, I give a detailed exposition of Heidegger’s analysis of the phenomena of mourning and loss and show that Heidegger’s analysis of mourning in his early and (...) late work is strikingly in line with his collectivist understanding of Being-with. This demonstrates, contrary to what some of his proponents argue, that Heidegger does not follow the kind of dynamic understanding of Being-with that places the other within fine-grained spaces of possibility. Second, with reference to Heidegger’s existential philosophy, I construct a phenomenology of mourning and grief. Though Heidegger himself fails to explain the relationships in which one mourns after a close other, we can develop a unique phenomenology of mourning with reference to Heidegger, which shows that each loss is singular and can be equiprimordial with one’s own death in opening one to the possibility of an authentic existence. In this new understanding of authenticity, loss is regarded as a powerful force, akin to death, in leading one toward their self-owned existence. (shrink)

Affects are intentional structures of beliefs and desires. Many philosophers have plausibly argued that Spinoza’s theory of ideas is a kind of theory of belief. Yet this claim has rarely been taken into account when it comes to Spinoza’s theory of affects, which is actually a part of his theory of ideas. This paper shows that if this point is taken seriously when regarding Spinoza’s theory of affects we reach significant results about the fifth part of Ethics. To show this, (...) I shall strive to show that all affects depend on some sort of beliefs by analyzing Spinoza’s theory of affects in terms of his theory of ideas, and in particular an affirmation which an idea naturally involves. From this revelation, we will be able to see that Spinoza’s theory of affects presented in the third and fourth parts of Ethics is inconsistent with the fifth part of Ethics in so far as the three therapy methods he describes at the beginning of the fifth part of Ethics are considered. I will also show that arguments by which the soundness of these therapy methods is guaranteed seem actually logically invalid. Finally, I will try to revise Spinoza’s therapy methods by taking all errors and core ideas in Spinoza’s theory of affects into consideration. (shrink)

Schematism has been generally criticized or deemed incomprehensible by thinkers, but it constitutes a vitally important aspect of the critical philosophy. In this study, I will attempt to show how schematism provides transitions between representations and establishes the relation between sensibility and understanding, also between understanding and reason. The relation is about how the understanding constructs the material of sensibility, and how the reason regulates the cognitions of the understanding. In this respect, I will also show that possible transitions between representations (discursive, determinative, reflexive and analogical) enable different ways of schematism. It will be shown that schematism grounds mathematics, and physiology (physics and metaphysics) based on temporality. (shrink)

Patients with either hereditary or acquired thrombophilia can present with arterial and venous thrombotic complications. However, it is unclear to whom the thrombophilia panel should be assessed, particularly in patients presenting with a common cardiovascular risk factor and acute coronary thrombus. Herein, we presented the management of an active smoker female patient who presented to our emergency room with inferior acute ST-segment elevation myocardial infarction, and hereditary thrombophilia has been diagnosed due to the presence of substantial left main coronary artery (...) thrombus. The theory of science that Thomas Kuhn built in the Structure of Scientific Revolutions was considered as a hypothetical framework in this study. Since the publication of the work, many questions have arisen that call for a psychology of science. These questions are moved to another dimension through the knowledge of the decision made within Galileo Affair, which occupies an important place in modern science, fundamentally arising from an epistemic struggle and emerging out of an unscientific base rather than the (...) charge of unholiness. Abandoning the perspective which evaluates these questions within a historical process as a weak side of the Kuhnian theory of science, this study challenges the current approaches with an alternative approach. The epistemic complexity in the Kuhnian theory of science is an imperative complexity caused by human factors. From this perspective, there are potential questions for psychology of science as a field of study based on Kuhnian epistemology and it can be assumed that new problems may appear when the other epistemological questions which assumed as “answered” are reviewed in the scope of this study. The main thesis of this study is that psychology of science is possible as a valid and operationalizable research program based on Kuhnian theory of science. (shrink)

Background -/- Retraction is a correction process for the scientific literature that acts as a barrier to the dissemination of articles that have serious faults or misleading data. The purpose of this study was to investigate the characteristics of retracted papers from Kazakhstan. Methods -/- Utilizing data from Retraction Watch, this cross-sectional descriptive analysis documented all retracted papers from Kazakhstan without regard to publication dates. The following data were recorded: publication title, DOI number, number of authors, publication date, retraction date, (...) source, publication type, subject category of publication, collaborating country, and retraction reason. Source index status, Scopus citation value, and Altmetric Attention Score were obtained. Results -/- Following the search, a total of 92 retracted papers were discovered. One duplicate article was excluded, leaving 91 publications for analysis. Most articles were retracted in 2022 (n = 22) and 2018 (n = 19). Among the identified publications, 49 (53.9%) were research articles, 39 (42.9%) were conference papers, 2 (2.2%) were review articles, and 1 (1.1%) was a book chapter. Russia (n = 24) and China (n = 5) were the most collaborative countries in the retracted publications. Fake-biased peer review (n = 38), plagiarism (n = 25), and duplication (n = 14) were the leading causes of retraction. Conclusion -/- The vast majority of the publications were research articles and conference papers. Russia was the leading collaborative country. The most prominent retraction reasons were fake-biased peer review, plagiarism, and duplication. Efforts to raise researchers’ understanding of the grounds for retraction and ethical research techniques are required in Kazakhstan. (shrink)

Objective: The use of neoadjuvant chemotherapy (NACT) in breast cancer is increasing. The management of locally advanced breast cancer differs due to the approach of the center to which the patient applied and the approach of the following physician. From this point of view, we aimed to evaluate the real life data of our region. -/- Methods: The study included 106 patients treated with neoadjuvant chemotherapy in the medical oncology clinic of two different university hospitals. Association between clinicopathological features and (...) pathological complete response (pCR) were analyzed. -/- Results: The pCR rate was higher in patients with negative hormone receptors and this difference was statistically significant (p:0.000). The rate of obtaining pCR increased as the NACT duration increased, and this increase was statistically significant. The mean NACT duration applied to the patients with pCR was 5.48 ± 0.22 months, and the mean NACT duration for those who could not obtain pCR was 5.01 ± 0.1 months (p:0.041). The recurrence rate of patients with pCR was 11.1%, while the recurrence rate of patients who could not obtain pCR was 31.6% (p:0.04). -/- Conclusions: Pathological response to chemotherapy is an important factor in determining prognosis. There appears to be a need for new biomarkers that allow the prediction of pCR and long-term outcomes. (shrink)

Coronary artery vasospasm (CAVS) is a clinical entity that can cause angina, but also unstable angina pectoris, acute myocardial infarction, fatal arrhythmias, and sudden death. Although it is a condition that is usually controlled with medical treatment, more aggressive treatments may rarely be required. In this case, the patient with a known diagnosis of CAVS had multiple arrests despite optimal medical treatment. We observed that fatal arrhythmias persisted in the Implantable Cardioverter Defibrillator (ICD) records, even though we implanted a stent (...) and gave the patient maximal medical treatment. We performed sympathectomy as a last resort and we did not detect any recurrence in the 6-month follow-up of the patient. ICD implantation and sympathectomy should always be considered in resistant CAVS cases. (shrink)

Objectives: Inferior oblique muscle overaction (IOOA) is a common ocular motility disorder. Ocular Vestibular Evoked Myogenic Potentials (oVEMP) are tests that evaluate the reflex pathway between the utricular macula and the inferior oblique muscle to detect vestibular diseases. Our study is of great importance as it is the first study in the literature to evaluate the effect of inferior oblique muscle overaction on oVEMP parameters. -/- Methods: Thirty-five patients with unilateral inferior oblique muscle overaction (IOOA group) and 18 healthy volunteers (...) without any neurological or vestibulocochlear disease were included in this study. All patients and healthy volunteers were evaluated with oVEMP. -/- Results: No statistically significant difference was found between the n1 latency, p1 latency, n1-p1 latency measurement values of the participants included in the study (p>0,05). A statistically significant difference was found between the n1-p1 amplitude measurement values of the participants in patient groups (non-squint eyes, squint eyes) and control groups (p-value was 0.038). -/- Conclusion: In IOOA patients, vestibulo-ocular reflex pathway may be affected, vestibular symptoms may develop thus o-VEMP responses may be affected. A careful anamnesis should be taken in IOOA patients, and it should be kept in mind that n1-p1 amplitudes and asymmetries may be significantly higher when o-VEMP is performed. (shrink)

Objective: The nutrient artery which enters through the nutrient foramen (NF) provides blood circulation and nutrition in long bones. This supply is essential during the growing period, the early phases of ossification, and in some surgical procedures. This study aimed to investigate NF in adult human ulnas in the Turkish population. -/- Methods: For this study, 155 (70 right and 85 left) Turkish dry adult human ulnas were used. The presence, number, and patency of NF were recorded as well as (...) its topography and direction. The vertical distance between the most proximal point of the olecranon and the proximal edge of the NF (DONF), and the longitudinal distance between the most ventral point of the coronoid process on the sagittal plane and the proximal edge of the NF (DCpNF) was calculated. Additionally, the foraminal index (FI) was assessed. -/- Results: Single and double NFs were in 139 ulnas (89.67%), and 3 ulnas (1.94%), respectively. NFs were not observed in 13 ulnas (8.39%). The majority of NFs (93.12%) were situated on the anterior surface of the ulna. The direction of all NFs was towards the elbow. The mean DONF and DCpNF were 9.48 ± 1.57 cm, and 6.68 ± 1.44 cm, respectively. The FI was 37.45% in ulnas with a single NF, while it was 41.46% in ulnas with a double NF. -/- Conclusion: Our study has presented additional information such as the FI of ulnas with 2 NFs, and the distance between the coronoid process and NF in the Turkish population. Objective: Primary care, which is often the first level of contact for patients with various communicable diseases (CDs) and noncommunicable diseases (NCDs), might exhibit patterns of injectable drug utilization different from hospitals. We aimed to examine injection prescribing to adults with CD or NCD in primary care. -/- Methods: In this retrospective study, single-diagnosis injectable drug-containing prescription data from Family Medicine Information System comprising 32 provinces of Turkey were analysed. The prescriptions were grouped by diagnosis as “CD” (n=3848) and “NCD” (...) (n=9338). Injectable drug utilization patterns in these groups were analysed by demographics, diagnoses, and drug subgroups. -/- Results: Out of 13186 prescriptions, 70.8% were issued for NCDs. NCD prescriptions were mostly generated for women and elderly (p<0.05 for both). About 63.3% (n=2948) of injectable drugs in CD prescriptions were antibiotics and 12.6% were analgesics. Cefazolin (15.2%) was the most commonly prescribed antibiotic for acute pharyngitis and acute sinusitis, and benzathine benzylpenicillin (12.8%) was the top-choice for acute tonsillitis and rheumatic fever. In NCD prescriptions, 34.0% (n=4214) of injectable agents were analgesics and 16.9% were muscle relaxants. The most frequently encountered drug in NCD prescriptions was thiocolchicoside (16.3%), which was the top-choice in all seven common musculoskeletal diagnoses. -/- Conclusion: Muscle relaxants and analgesics were the most commonly prescribed injectable drugs for NCDs, musculoskeletal diseases in particular. Antibiotics were frequently encountered in CD prescriptions, mostly as broad-spectrum for lower respiratory tract infections (RTIs) and narrow-spectrum for upper RTIs. These findings may elucidate the issues to especially focus on regarding excessive use of injections. (shrink)

Even though in both cases the psychological state that underlies the behavior is called ‘trust’, it is controversial whether it is a token of the same psychological type. Trust can serve an affective, epistemic, or other social function, and comes to interact with other psychological states in a variety of ways. The way that the functional role (...) of trust changes across contexts and objects is further complicated when communities and individuals mediate it through technologies, and even more so when that mediation involves artificial intelligence (AI) and machine learning (ML). In this chapter I look at the ways in which trust in institutions, and specifically the medical profession, is affected by the use of AI and ML. There are two key elements of this analysis. The first is a disanalogy between institutional trust in medicine and institutional trust in science (Irzik and Kurtulmus 2021, 2019; Kitcher 2001). I note that as AI and ML become a more prominent part of medicine, trust in a medical institution becomes more like trust in a scientific institution. This is problematic for institutional trust in medicine and the practice of medicine, since institutional trust in science has been undermined by, among other things, the spread of misinformation online and the replication crisis (Romero 2019). There is also a strong analogy between the psychological state of the person who trusts a scientific report or testimony and the psychological state of a patient who trusts individual recommendations made by a medical professional in a clinical setting. In both cases, institutional trust makes it less likely that a mistake or malfeasance will result in reactive attitudes, such as blame or anger, directed at other individual members of that institution. However, it also renders people vulnerable enough to blame the institution itself. This, with time, can erode trust in the institution and naturally leads to policy recommendations that aim to preserve institutional trust. I survey two ways in which that can be done with institutional trust in medicine in the age of AI and ML. (shrink)

There are a lot of studies on Ahmed Cevdet Paşa who has been considered as statesman, politician, jurist, historian, sociologist and educationalist. At the same time Ahmet Cevdet Paşa’s studies on logic are critically important when we deal with the last period of the Ottoman Empire. His book which is called Miyar-ı Sedad had both influenced the development of instruction of logic education in madrasahs and created a turning point in the studies on modern logic. After the death of Mevlana Celaleddin-i Rumi (d.1273), the cult of Mevlevi continued its structuring. Rumi's son Sultan Veled (d.1312) and his grandson Ulu Arif Çelebi (d.1320) contributed greatly to this structuring. Sultan Veled tried to turn the lodge he took over from his father into a systematic sect around Rumi's mystical thought and Mevlevi disciples. Ulu Arif Çelebi, on the other hand, is a very effective name in organizing Mevleviism as a cult. With his systematic studies and travels, he (...) made Mevlevi's spread in lodges and madrasahs, and he made efforts to teach Masnavi, the work of his grandfather Mevlana Celaleddin Rumi, in madrasahs. In addition, Ulu Arif Çelebi asked Ahmet Eflakî to write a book called Ariflerin Menkıbeleri about Rumi’s life, in order to keep his grandfather's memory alive. The entry of Mevlevi into the Ottoman State was through marriage. Mutahhare Hatun, daughter of Sultan Veled, was married to Süleyman Şah, the Germiyanoğulları leader. Süleyman Şah married the daughter of this marriage, Devlet Hatun, to Yıldırım Beyazıt, and left as a dowry Kütahya, Tavsanlı, Simav and Eğrigöz to Ottoman Empire. Yıldırım Bayezıd was appointed as the governor of Kütahya. Mehmet Çelebi was born from this marriage. The title of Çelebi is a title given to the descendants of Rumi, and Mehmet Çelebi, one of the sultans of the Fetret Period (A period of great depression), was one of the grandchildren of Rumi. The children of Mehmet Çelebi are remembered with the title of 'Çelebi'. Through this marriage, Mevlevi began to spread to the Ottoman Empire. Thus, Mevleviism would prevail in the Ottoman Empire, where Ahilik organization and Ahi culture prevailed, and Mevlevihane and Asitane would be established in many parts of the country with the support of the Ottoman State. Mevlana Celaleddin-i Rumi’nin (ö.1273) ölümünden sonra Mevlevilik bir tarikat olarak yapılanmasını sürdürmüştür. Mevlana’nın oğlu Sultan Veled’in (ö.1312) ve torunu Ulu Arif Çelebi’nin (ö.1320) bu yapılanmada katkısı oldukça büyüktür. Sultan Veled babasından devraldığı dergâhı, Mevlana’nın mistik düşüncesi çerçevesinde ve Mevlevi müritler etrafında sistemli bir tarikat haline getirmeye çalışmıştır. Ulu Arif Çelebi ise Mevleviliği bir tarikat olarak örgütlemede oldukça etkili bir isimdir. O sistematik olarak yaptığı çalışmalar ve seyahatlerle Mevleviliğin tekke, zaviye ve medreselerde yayılmasını sağladığı gibi dedesi Mevlana Celaleddin ’in eseri Mesnevi’nin medreselerde okutulması için de çaba sarf etmiştir. Ayrıca Ulu Arif Çelebi, dedesinin hatırasını canlı tutabilmek Ahmet Eflaki adındaki bir yazıcıya Mevlana’nın hayatını yazdırmıştır. (Ariflerin Menkıbeleri kitabı) Mevleviliğin Osmanlı Devleti’ne girişi evlilik yoluyla olmuştur. Sultan Veled’in kızı Mutahhare Hatun, Germiyanoğulları Beyi Süleyman Şah ile evlidir. Süleyman Şah bu evlilikten olan kızı Devlet Hatun’u Osmanlı Devleti’nin o dönem şehzadesi olan Yıldırım Beyazıt ile evlendirmiş ve çeyiz olarak Kütahya, Tavşanlı, Simav ve Eğrigöz dolaylarını Osmanlılara bırakmıştır. Yıldırım Bayezıd ise Kütahya valiliğine getirilmiştir. Bu evlilikten Mehmet Çelebi dünyaya gelmiştir. Çelebi unvanı Mevlana’nın soyundan gelenlere verilen bir unvandır ve Fetret Dönemi padişahlarından olan Mehmet Çelebi, Mevlana’nın torunlarındandır. Mehmet Çelebi’nin çocukları ‘çelebi’ unvanıyla anılmaktadır. Bu evlilik yoluyla Mevlevilik Osmanlı’da yayılmaya başlamıştır. Böylece daha önce Ahilik teşkilatı ve Ahi kültürünün hâkim olduğu Osmanlı Devleti’ne Mevlevilik hâkim olacak ve Osmanlı Devleti’nin desteğiyle ülkenin birçok yerinde Mevlevihane ve Asitane kurulacaktır.
